T4 Physio Limited Company, is located  at  69 Bolton Road, Kearsley, BL4 8DB and our website is www.t4physio.co.uk.We offer health services to treat health problems and give a diagnose where possible.  All treatments are governed by the Chartered Society of Physiotherapy (CSP). T4 Physio is regulated by the Health and Care Professions Council (HCPC). The practice also provides holistic therapies provided by other staff members and they would be please to provide more details about the treatments.

2 – Personal Data- The information we collect and use

All personal information collected via website(s), including your email address and other personal information provided to use through our services or contacting us for information regarding our services. All information provided to use will stored and used solely for the purposes of fulfilling your requests for information about our services, delivering a request through the website or communicating with you as a member of the service.  All the information you provide is always held securely, is not shared with anyone not involved in your treatment, although for data storage purposes it may be handled by pre-vetted staff who have all signed an integrity and confidentiality agreement.

 3 – Retention Policy

T4 Physio will process personal data during the duration of any treatment and will continue to store only the personal data needed for eight years after the contract has expired to meet any legal obligations. After eight years all personal data will be deleted.

4 – Information Recording

We may keep copies or recordings of any correspondence you have between us, including emails, telephone calls, consultations, CCTV and voice recordings. This may be used for quality and training purposes, and to prevent criminal behaviour.

 5 – Complaints
In the event that you wish to make a complaint about how your personal data is being processed by the Clinic you have the right to complain to us. If you do not get a response within 30 days, you can complain to the ICO. 6 – Unsubscribe/Opt-out Information

These options are made available when you sign-up for our email lists and in email messages delivered from our company. Alternatively, you may email us at info@t4physio.com with the subject title: Remove and you will be permanently removed from our mailing list.

Please check this Privacy Policy document as we might update it from time to time.
